I think the problem may be with the OFF file:

> 	3.0 0.0 0.0
> 	0 1 2 3 4 5

The last line --- the one giving the one face for this
OFF object, should begin with an integer indicating the number
of vertices in that face.   I just tried

OFF
6 1 6
	0.0 0.0 0.0
	0.5 1.0 0.0
	1.0 0.0 0.0
	2.0 0.0 0.0
	2.5 1.0 0.0
	3.0 0.0 0.0
	6 0 1 2 3 4 5

and it seems to be fine.
