As for a source release, we're working on one.

As for the stereo module, the distributed binary was compiled under Irix 3.3;
it looks as though the Indigo Extreme's backward-compatibility library
doesn't handle the setmonitor() call, which enables hardware stereo.
We've just gotten an Indigo Extreme, and it fails just as yours does with the
old binary, but works with a new 4.0 binary.

I've put a new binary for the 'stereo' module up for anonymous FTP, pending
a real release; you can grab it from geom.umn.edu under the "slevy" directory.
