The problem is with your OFF header information. The format for OFF is:

OFF
NVERTICES NFACES NEDGES

In your case
NVERTICES = 12
NFACES = 4 (not 36)
NEDGES = 6 (not 99, though this number can be anything since
		geomview doesn't use it for anything)

Thus, the header should be:

OFF
12 4 6
